Registration must be completed within 30 days of the start of the sports season.
Physical Examination
Students registering for sports must have a current physical examination form submitted to the health office for review before the student is authorized to participate in practice or tryouts. Click the Athletic Team Registration icon and complete a family account and follow the registration instructions. Please contact your child's health care provider to get orders for your child to self-carry and self-administer any emergency medication they may be taking such as inhalers and epi-pens. Physicals and medication orders should be faxed to the HS Nurse at 518-565-5809 or to the MS Nurse at 518-565-5706.
